Preserving Sensation and Function: The Pedicle Technique
A key consideration in breast reduction surgery is maintaining blood supply and nerve function. At the heart of most procedures is what we call the pedicle technique – a careful surgical approach that preserves sensation and natural breast function. The pedicles act as a lifeline, carrying essential blood supply and nerve connections to your nipple and areola. Various pedicles produce different results in aesthetic appearance. Based on your breast anatomy, tissue composition, and desired outcome, we may recommend different pedicle positions:
- Inferior Pedicle – Blood and nerve supply come from the bottom of the breast.
- Superior Pedicle – Blood and nerve supply come from the top of the breast.
- Central Pedicle – Blood and nerve supply come from the centre of the breast.
- Lateral Pedicle – Blood and nerve supply come from the sides of the breast.
- Bi-Pedicle – Blood supply comes from two locations, typically used in reconstructive surgery.
In the past, breast reduction mammoplasty was commonly performed using the inferior pedicle technique, where the blood supply to the nipple came from the lower part of the breast. While effective in reducing breast size, this approach often left the lower breast bulkier, leading to a boxy appearance and an increased likelihood of sagging over time.
Today, we use more advanced techniques such as the superior, superomedial, or superolateral pedicle methods, where the blood supply is preserved from the upper or outer regions of the breast. This allows for the removal of excess lower breast tissue, creating a more natural, lifted shape while also reducing the chances of future sagging.
No matter which option of the three above is chosen, nipples and areolas are never detached from their blood and nerve supply at any point. However, for patients with excessively large breasts, a Free Nipple Graft (FNG) procedure may be required. In such cases, the nipple-areola complex is detached and repositioned, which results in permanent loss of sensation and the inability to breastfeed.

Incision Methods to Minimize Scarring
At BB Clinic, we may choose from three specific incision methods based on the patient’s breast size and shape, the amount of skin or tissue that needs to be removed, position of the nipples and the quality of the skin. In each of these, a pre-determined amount of fat will be removed from your breasts to decrease their size, followed by the necessary reshaping to create the look you desire.
Vertical Scar (“Lollipop” Incision): A common technique for moderate breast reductions, involving an incision around the areola and vertically down to the crease. This method results in less scarring but may not be suitable for very large breasts.
Wise Pattern Scar (“Anchor” Incision): The gold standard for larger reductions, this incision allows for more tissue removal and reshaping while ensuring a natural, lifted appearance. Excess skin, fat, and tissue are removed with the nipple then being moved to a higher position with sticking that reshapes the breast. We now know that an anchor-type incision utilising a superior rotary pedicle internally achieves the best results for actual breast reductions.
Round Block / Benelli Technique (“Donut” Incision): A minimal-scar approach that involves an incision around the outer edge of the areola. This method is best for smaller reductions with minimal excess skin, and has been known to create a flat breast.
