A lift alone is usually enough when:

• You’re happy with your current breast volume/cup size
• The main concern is position (nipples pointing downward, breasts appearing deflated or elongated rather than “empty”)
• You have enough of your own tissue to fill the reshaped skin envelope

In these cases, a breast lift without implants (or a breast lift with fat transfer for modest, natural-feeling upper-pole fullness) can achieve a lifted, proportionate result using your own tissue.

Implants are typically added when:

• There’s noticeable volume loss, particularly in the upper pole, alongside sagging
• You want a fuller cup size, not just repositioning
• Skin laxity is significant enough that volume is needed to support long-term shape retention

Combining a lift with implants or fat transfer in a single operation can reduce total time under anaesthesia and simplify recovery compared to staging the procedures separately. It does affect surgical planning and cost, so it’s worth discussing all options with your surgeon at your consultation.

Boob lift surgery is performed under general anaesthesia and can be done as day surgery or with overnight admission, typically taking 2–3 hours. Most patients return to work in 7–14 days, with light exercise resuming after about one month.

Breast lift results are long-lasting but not permanent. Gravity, weight change, and future pregnancies will continue to influence breast shape over time, regardless of technique. Published surgical literature places long-term revision rates for mastopexy in Sydney in the region of 15–20%, most commonly related to recurrent ptosis (sagging) in patients with heavier breasts, significant weight fluctuation, or skin of lower elasticity. 

Maintaining a stable weight and following post-operative garment guidance are the most effective ways to protect your result. Your surgeon will discuss your individual risk factors during consultation.

We generally recommend waiting until breast development, childbirth, and breastfeeding are complete before undergoing surgery. While it’s possible to lift breasts of any size, results tend to be less durable in patients with heavier breasts.

A mastopexy without implants may suit you if:

• You are physically healthy
• You have realistic expectations
• Your breasts are fully developed
• Your breasts are sagging and have lost shape and firmness
• Your breasts have a flatter, elongated, or pendulous shape
• Your nipples and areolas point downward
• You have stretched skin and enlarged areolas
• One breast sits lower than the other

A lift will not significantly change breast size, as it reduces the skin envelope around existing tissue. If you’d like a fuller or smaller result, ask about combining your lift with breast augmentation or breast reduction.
