Not all of the fat that’s injected into the breast stays there permanently. Some of it establishes a new blood supply and survives long-term. The rest is gradually reabsorbed by the body over the following months. Fat survival generally ranges from 60–70% of the injected volume, though this varies by patient, technique, and how the fat is handled during processing.

• The volume you see in the breast at six months is a reasonable indicator of your result, because reabsorption has largely settled by that point.
• Your surgeon will typically inject more volume than the target size, anticipating that a portion will reabsorb.
• Fat that doesn’t survive is either reabsorbed smoothly or, less commonly, breaks down unevenly, which can create firm areas or lumps (fat necrosis). 
• Because survival isn’t 100%, fat transfer breast augmentation typically produces a more modest size change than implants.

Your surgeon will talk you through expected survival rates based on your anatomy, the donor site used, and the volume being transferred, so you have a realistic picture before proceeding.

Because fat grafting has a volume ceiling, one of the first decisions in your consultation is which category of breast augmentation actually fits your goals.

Fat Transfer Only

Suited to patients after a subtle, modest increase, typically within about one to one and a half cup sizes, who want to avoid implanted material altogether and have enough donor fat available to harvest. This approach also gives you body contouring at the donor site as part of the same procedure.

Implants Only

The better fit if you’re after a larger or more defined size increase than fat grafting alone can deliver, don’t have sufficient donor fat, or want a specific, predictable shape and projection that isn’t dependent on fat survival rates. Combined Fat Transfer and Implants

Fat can be layered over or around an implant to soften the edges, fill in areas of thin tissue coverage, or correct rippling and asymmetry, while the implant provides the bulk of the increase in size. This is a common option for patients who want implant-level volume with some of the contouring benefits of fat grafting.

The right choice depends on your donor fat, your goals for size, and your tolerance for a procedure that may need a second round to reach the final result. Not always. Because a portion of the transferred fat is reabsorbed, the result after one session may sit below your original target, particularly if you’re seeking a larger increase or have limited donor fat available. Many patients get the size and shape they want from a single procedure. Others, especially those seeking a fuller result, choose a second fat transfer session several months later, once the first round has fully settled, to build on the surviving volume.

If you know going in that you want a larger, more definitive size increase, it’s worth discussing with your surgeon whether a single round of fat transfer breast augmentation in NSW will realistically get you there, or whether staging two sessions, or combining fat transfer with an implant, is the more direct route. 

If you’re a candidate for fat grafting, it’s worth understanding how the procedure can affect breast imaging down the track.

Fat grafting can produce benign changes inside the breast, most commonly oil cysts and small areas of calcification, as the body processes any fat that doesn’t survive. These changes have a recognisable appearance on mammography and are generally distinguishable from breast cancer findings by an experienced radiologist. They’re a known and well-documented feature of fat grafting, not a sign that something has gone wrong.

• Tell your radiologist. Always disclose that you’ve had fat grafting to the breast before any future mammogram, so the images can be read with that context in mind.
• A baseline mammogram helps. Having imaging done before surgery, and again once your results have settled, gives your care team a clear “before and after” reference for any future screening.
• Screening isn’t interrupted. Fat grafting doesn’t prevent you from undergoing routine breast screening; it simply means your radiologist needs to know your surgical history.

This is a conversation worth having directly with your surgeon during consultation, particularly if you have a family history of breast cancer or attend regular screening.

This procedure may not suit you if you’re seeking a dramatic increase in size, have very little donor fat to work with, or are primarily looking to correct significant sagging, in which case a lift may be more appropriate. Many women also explore fat transfer as part of broader post-pregnancy procedures or a mummy makeover surgery, where breast volume loss is combined with changes elsewhere in the body.

Recovery timelines vary from patient to patient depending on the areas treated, overall health, and how closely aftercare instructions are followed. Because two sites are involved, both the breasts and the donor site need care during healing.

Most patients take at least one week off work. Swelling typically eases significantly within four to six weeks, though it can take several months for the breasts to fully settle into their result.
